Report into the System for Dealing with Multiple Child Deaths
On 28 August 2003, the Premier, the Hon Steve Bracks, MP, requested a report on the adequacy of the system in place for dealing with child deaths and recommendation as to how the system can be improved.
This report describes the current processes for handling child deaths, analyses these processes with the problem of multiple child deaths in mind, and provides the key recommendations for systems change including some details about the changes to legislation and administrative practice that are recommended.
